2027 Governor’s Arts Award Nomination Form

The nomination form and required attachments are expected to be a clear representation of the nominee’s strengths and contributions to the arts in Tennessee and should reflect why the individual, organization, community, or business is being nominated for a specific category. Reminder: You may only submit one form. Multiple nominations are not eligible. How to apply: Complete the online nomination form or download the form and complete with Acrobat Reader and print a hard copy. If completing online, upload the documents below when prompted. Be sure to save and submit. If mailing, submit the hard copies of the following documents with the printed form and mail to: Governor’s Arts Awards Committee Tennessee Arts Commission 320 6th Ave N. If nominating for the Distinguished Artist Award: Describe how the nominee has exceptional talent and/or creativity in one or more artistic disciplines. If nominating for the Folklife Heritage Award: Describe how the nominee demonstrates significant achievement within an art form that is rooted in the traditional or ethnic cultures of Tennessee.
  • If nominating for the Arts Leadership Award: Describe how the nominee’s impact on or advancement of the value of the arts in Tennessee. If nominating for the Distinguished Artist Award: Describe how the nominee has made a significant and lasting impact on a state or national level. If nominating for the Folklife Heritage Award: Describe how the nominee demonstrates outstanding contributions to preserving and promoting traditional arts in Tennessee.
